has multiple SSNs refer the AU member to the SSA to resolve the discrepancy; enter all known numbers into the system; inform the A/R of his or her responsibility to report the correct and primary SSN to the county office upon resolution with SSA
never had an SSN
or
had an SSN but the number is unknown or cannot be located
refer the AU member to SSA to apply for a new or replacement SSN and follow the steps listed below:
  1. A/R must submit original or certified copies of documents that verify age, identity, and citizenship to SSA with the application for an SSN
  2. issue a verification checklist (SUCCESS generated or Form 173), allowing ten calendar days for Form SS-5 to be completed and returned (by either the A/R or the SSA)
  3. document completion of the enumeration process when a completed referral form or other SSA documentary evidence is received
is enumerated at birth by a medical facility verify the application for an SSN using a Record of Live Birth (Form SSA 2853) and enter a future action to check for receipt of SSN at next review
