The table below indicates what type of verification is required for basic eligibility criteria and income sources. Written or verbal information from a reliable third party is acceptable third-party verification. Third-party verification can always be requested in place of an applicant's statement when the agency has received conflicting information.

Dependent care expenses	Statement from dependent care provider (1615).
Deprivation--due to death	AU's statement unless questionable (1316).
Deprivation--due to continued absence	AU's statement, unless questionable (1317).
Deprivation--due to incapacity	Numerous, as specified (1318).
Deprivation--due to recent connection to the work force	AU's statement, unless questionable (1319).
Enumeration, if SSN is available	AU's statement, unless questionable (1325).
Enumeration, if application is made	Proof of application (1325).
Immunization	DHS Form 3231, Child Care Immunization Certificate, or other verification that includes the same information as the 3231 (1360).
Income	Pay stubs, award letter and other sources, as specified (1525).

Resources	Numerous sources, as specified (1505).
School Attendance	AU's statement for children under 18, unless questionable. For an 18-year-old dependent child, attendance must be verified as specified (1347).
Terminated Income	Statement from former employer (1525).
